Please complete our New Client Package below and return it to us along with the 2 items listed below ONLY IF you have lived at your current address for less than 1 year. (We only need your proof of social and proof of address if you changed your mailing address in the past year). These items include:
- This completed & signed agreement (Pages 3 and 4 below)
- If you have moved in the past 6 months (and only if), we will need your proof of social security and proof of current mailing address. (valid social proof is a copy of your social security card, a W2 or a 1099) and (valid address proof is a utility bill that shows your current mailing address and your name or a driver’s license or State I.D. if it shows your current mailing address.

If you haven’t already, please get your 3-Bureau credit report and scores using our discount link for Identity iQ at https://bit.ly/jamisoniq We discount our monthly service fee by $22 in order to completely cover the cost of IdentityIQ for you. We need IdentityIQ in order to login and update your 3 credit reports every 40 days to see what progress was made. Our service is much more effective whenever we are able to pull an updated credit report without relying on the credit bureaus to send updates. We look forward to working with you. This agreement is in response to client's desire to retain Jamison Law Group for professional credit services.
ITEMS NOT TO BE NEGOTIATED
For Clients that opt to use our dispute service, the undersigned agrees as follows. I (the client) believe that all negative items which appear on my consumer credit reports may not be accurate, timely, or verifiable and I want Jamison Law Group to attempt to get them deleted or verified as being accurate. I agree to email a list of the negative items that I feel are 100% accurate and verifiable to email@example.com. I understand that the items I email to that email address will not be disputed or attempted to be removed and will remain. (If you do not email a list of any items, and if you told us you wanted us to dispute your negatives, we will assume that all negative items are inaccurate and should be disputed.)
Jamison Law Group will assume that the reason you are asking us to dispute the negative items that appear on your credit reports falls under one or more of the following categories:
- I do not recall these listings as they are being reported
- I do not believe these listings are being reported accurately
- I think that I may be a victim of identity theft
- The status of these listings isn’t accurate of they don’t belong to me
- I do not believe these listings are verifiable
- I believe there is a file mix-up with someone else
- I was never late on this account
Unless Client tells us otherwise via email to firstname.lastname@example.org, Client agrees that we may share updates with their loan officer regarding the progress of their credit improvement if that loan officer was mentioned in the “referred by” optional field below.
Attorney Client Relationship
If in the future Jamison Law Group and Client decide to pursue a lawsuit against the credit bureaus and/or creditors, client would need to sign a separate retainer agreement with an Attorney that may or may not be employed by Jamison Law Group. This current agreement in no way creates an Attorney Client relationship between Client and anyone at Jamison Law Group. A separate agreement would need to be drafted and signed to create an Attorney/Client relationship and client agrees that this agreement does not create that Attorney Client relationship in any way.
Fees for Services/ Cancelations
Client agrees to allow Jamison Law Group to charge their credit card or debit card for the agreed upon fee of $177 per month “per client” for months 1 to 3, $77.50 per month for months 4 to 6, and $27.75 per month for months 7 and beyond (Our regular monthly price of $199, $99.50 and $49.75 is discounted by $22 to reimburse client for IdentityIQ). We charge the 1st payment at the moment of first work performed (usually the same day) and an additional $177 per month for months 2 and 3 and then $77.50 for months 4 thru 6, and then $27.50 for months 7 and beyond every 30 days until client cancels. Client can cancel at any time by sending an email with the subject line stating “CANCEL” to
email@example.com. Client agrees that any charges made up to and before Client sends this cancellation email are considered earned in full and will not be refunded.
Canceling will prevent all future charges to Client.
